Looking for a little help identifying this plate which, as you can see, is broken. I was hoping to find a replacement but know nothing about it. There are no visible markings on the back as it is covered by a hanging sticker.
Any help at all would be greatly appreciated. Thanks in advance

Suggest that you remove the hanging sticker!!!
Several ways to remove: Hair dryer - blow hot heat on for about a minute then try and remove.
Other methods: nail polish remover, Goo Gone, Rubbing Alcohol, etc.
Really need to see markings on the back!!!
