Not sure about middle name but looks like "Sue Nora Canski or Censki' Hand made by Old Settlers, Pine Az, Isaiah, 64:8" 168 of a 500 edition !!

I do know that if you search Pine, AZ there is lots of information about the native indians and how they sold crafts there.
